Choice Health at Home has announced that it acquired three companies: Cy-Fair Health Care, Alliant Home Health, Palliative and Hospice Care as well as Senior Nannies Private Care.
The newly acquired companies span the in-home care continuum, continuing CEO David Jackson’s strategy of becoming a comprehensive in-home care provider.
“When personal care and skilled clinical services work together, patients experience better outcomes and a higher quality of life,” Jackson said in a statement about the acquisition of Senior Nannies.
Tyler, Texas-based Choice Health at Home provides home health, hospice, personal care and rehabilitation services in Texas, Oklahoma, Kansas, Arizona, Colorado, Utah, and Nevada. The acquisition of Tomball, Texas-based Cy-Fair Health Care expands the company’s Texas footprint.
Cy-Fair Health Care provides home health services, including wound care, occupational therapy, physical therapy and diabetic teaching programs across Southeast Texas.
The Alliant acquisition, meanwhile, expands Choice’s Denver, Colorado, footprint.
“Combined with Lumicare Hospice, which we acquired in 2024, and Family Tree Private Care in early 2025, we are now uniquely positioned to deliver all core levels of home-based care in the region with a level of quality, coordination and compassion that sets us apart,” Jackson said.
Alliant provides home health, hospice and palliative services, based in offices in Westminster and Colorado Springs, Colorado. Its home health services include skilled nursing, orthopedic rehabilitation, medical social services, occupational therapy and wound care.
Also acquired was Senior Nannies Private Care, affiliates of CWGL Holdings. Under the Senior Nannies umbrella are Senior Nannies Home Care Services, SN Home Healthcare and Senior Advantages Assisted Living Placements.

Fort Lauderdale, Florida-based Senior Nannies provides skilled nursing, personal home care and personalized assisted living placement solutions.
The Florida expansion is a “strategic anchor” for Choice, according to the company, and will be used to support further growth in the Southern U.S.
